Changing your details

If you're registered with Te Poari Ringa Hangarau Iraruke | The Medical Radiation Technologists Board (the Board), you are required to promptly notify the Board of any changes to your details, such as your name or address, as per section 140 of the Health Practitioners Competence Assurance Act 2003 (HPCA/the Act).

Even if you're not currently practising in your profession, if you're on our register, you must notify the Board of any change to your details.

Notice of a name change must be accompanied by a certified copy of your marriage certificate, deed poll, or statutory declaration. These documents must be certified as a true copy by a Justice of the Peace, or a solicitor, or an official authorised to make statutory declarations.
